As a Home Health Scheduler, you'll be the linchpin in coordinating our clinicians' schedules, ensuring that our patients receive the care they need, when they need it. Key Responsibilities Manage and coordinate clinician schedules on a weekly basis, ensuring optimal productivity and patient coverage. Enter patient data into our database, tracking frequency and duration of visits to ensure seamless care continuity. Collaborate with clinicians to determine the most appropriate care providers for patient visits, leveraging skills and availability. Communicate effectively with patients, providing them with visit schedules and addressing any concerns they may have. Track lab results and ensure timely dissemination to clinicians, physicians, and other relevant parties. Coordinate with staff regarding new patient assignments, ensuring a smooth onboarding process. Maintain accurate and up-to-date records, including patient information and agency census data. Prepare and distribute on-call information for weekends, ensuring that administrators and clinicians are informed and prepared. Set and coordinate schedules for Staffing Coordinators to ensure consistent weekend coverage and productivity.
